Gateway Tryout and Waiver Form

Forms Each player must complete both of the forms provided above. The Waiver Form must be signed both by the player and by a parent or legal guardian. Additional information is available in the Gateway Girls Tryouts FAQ document.

Here are the Gateway Region  2021-2022 Age Division Birthday Cutoffs.

Tryout Cost: $10

IMPORTANT
  1. 314 VBC does not accept waivered players. 
  2. If you have a conflict with another tryout, please contact us to find a mutually agreeable alternate tryout time.
  3. Doors will open for registration one-half hour prior to tryout time. Please try to get there early in order to register and to warm up.  
  4. ​Completing the forms prior to arrival will expedite the registration process.

18U - July 24 Time 12:00 - 1:30 PM

Gateway Tryout and Waiver Form

Please complete the registration form AND the Gateway Tryout Waiver above. The registration is on-line and does not need to be printed. The waiver must also be signed for each player both by the player and by a parent or legal guardian. Please print, sign, and bring with you to the tryout.
